Admission to Slaters Country Care requires the following reports, tests,
physician's orders and agreements. All the necessary forms to be completed
will be included within your admission packet.

Medical

• Completed physician's medical report.
• Physician's orders for all medications, including all over-the-counter medications.
• Medication account set up with a local pharmacy providing delivery service.
• T.B. skin test.

Insurance

Photocopy of medical insurance cards.

Personal Property

Please include a list of all the resident's personal items with all items labeled preadmission.
As we encourage the residents to make their space their own, those items may include
furniture, etc.

Admission Agreements

• Completed forms in the admission packet, including emergency contact information.
• Preadmission evaluation by Administrator
